Submit a Clinical Order

The PANDA Core for Genomics and Microbiome Research is CAP accredited (9755538) and CLIA certified (03D2289424). We are licensed to provide clinical diagnostic testing in 49 states (all States except the State of New York).

Submit Samples Online

We now use an online form for all sample submissions. To use it, register once, create a password, and receive your personal submission link by email.

One-time registration

Every submitter registers a single time before their first submission. Registration is quick and only needs to be done once.

1 Open the registration form and complete your submitter details.
2 Create a password to secure your account.
3 Check your email. Once you register, we email you a personal submission link.

Your personal submission link

After registering, you’ll receive a link that is unique to you. Use this same link every time you submit samples — there’s no need to register again.

What to know about your link

  • Save it. Keep your personal link somewhere you can easily find it — you’ll use it for every submission.
  • Monthly reminder. We resend your link once a month so it’s always within reach.
  • Can’t find it? Email PANDACore@arizona.edu and we’ll resend it.
  • Do not share your link. It is tied to your registration and is for your use only.

Collect and prepare your specimen following the Order and Shipping Instructions, then submit it using your personal link.
